We anticipate 320 worldwide attendees from academia, industry, and government.

Provided for each sponsor:   2 foot by 3 foot sign with company name. Upon request, information about your company may be included in the sign.

Contact information provided in your application will be included in the Proceedings Volume, web page, etc., if payment obligations are met.

DEADLINES:

March 1, 1999:   application for sponsorship due (see above form)

April 1, 1999:   notification of acceptance status

May 1, 1999:   payment due   (if sponsorship accepted)

• Payment methods accepted are:
• Check, bank draft, or money order payable to University of Kentucky.
• Government electronic transfer (forms must be initiated by sponsor)
• International bank transfer - please call (606) 257-2820 for account number. Sponsor is responsible for all transfer charges.
• Visa or Mastercard credit cards (We cannot accept other credit cards.)

• Payment must be in U. S. Dollars.

NOTE: PAYMENTS STILL OUTSTANDING BY AUGUST 2^ND WILL RESULT IN FORFEITURE OF INCLUSION IN THE PROCEEDINGS VOLUME, WEB PAGE, ETC.
