• Take advantage of the lower prices on or before September 4, 2001
• Payment MUST accompany registration form.
• Payment methods accepted are:
• Check, bank draft, or money order payable to University of Kentucky.
• Visa or Mastercard credit cards (We cannot accept other credit cards.)
• Electronic registration and registration by fax are allowed ONLY if using a credit card. Fax registration to: (859) 257-2173.
• Payment must be in U. S. Dollars.
• Please return completed form and payment to:

• Cancellation with refund (less 15% administrative fee) is available until October 15, 2001.
• No refunds will be made after October 15, 2001.
• Confirmation and receipt will be sent only upon request by notifying Geaunita Caylor at 859/257-2820 or email gcaylor@engr.uky.edu. Your receipt will be included with the symposium materials you receive at the registration desk.
• Substitutions may be made at any time by notifying Ms. Caylor at 859/257-2820. Please advise us in advance of any name changes.
• Each attendee will be provided with a booklet of abstracts with his/her symposium materials in order to facilitate his/her selection of presentations to attend.
• We cannot accept any credit card other than Mastercard or Visa.
