Registration Information

Take advantage of the lower prices on or before September 2, 2003.

Payment MUST accompany registration form.

Payment methods accepted are:

• Check, bank draft, or money order in U. S. Dollars made payable to University of Kentucky.
• Visa, Mastercard, Discover, or American Express credit cards (We cannot accept other credit cards.)

Registration by fax is allowed ONLY if using a credit card. Fax registration to: (859) 257-2173. There is no online registration.

Payment must be in U. S. Dollars.

We can not accept payment by electronic/bank transfer.

Cancellation with refund (less 15% administrative fee) is available until October 13, 2003. No refunds will be made after October 13, 2003.

Confirmation and receipt will be sent only upon request by notifying Geaunita Caylor at 859/257-2820 or email gcaylor@engr.uky.edu. Your receipt will be included with the symposium materials you receive at the registration desk.

Substitutions may be made at any time by notifying Ms. Caylor at 859/257-2820 or email gcaylor@engr.uky.edu. Please advise us in advance of any name changes.

Each attendee will be provided with a booklet of abstracts with his/her symposium materials in order to facilitate his/her selection of presentations to attend.
